Program 1
$7.5MAgrace's other programs include the volunteer services program, the food service program and grants to support related organizations. In 2024, 630 specially trained volunteers provided 69,158 hours of service. Because of this support, agrace is able to defer some staffing costs and continue to provide compassionate, expert care to patients and families. Some of the volunteer positions include in-home and residential patient and family volunteer; grief services, administrative office work, flower room and kitchen volunteer; vigil program; and agrace hospitality volunteer. Food service: agrace employs talented chefs to provide our patients and families comforting meals cooked with locally grown, often organic fresh produce, meats and herbs. Staff dietitians work with the kitchen staff to prepare meals that provide sustenance and comfort to patients. Sometimes a patient will not be able to digest solid foods, but small meals can be prepared for them to enjoy the comforts of the smells that meals provide. In addition to patient and family meals, agrace food services makes nutritious meals and snacks for clients at the agrace adult day center. Fresh baked goods, coffee and tea are available for purchase by staff, volunteers and guests at the doc rock cafe at our madison campus. In 2024, agrace, inc. Transferred funds totaling $5,700,000 to the risk reserve account held by agrace hospicecare foundation, inc.
Program 2
$2.6MAgrace HME, LLC is a disregarded entity that was established in 2019 to provide home medical equipment to Agrace hospice patients. With a fleet of delivery vans and a caring crew of DME technicians, Agrace delivers home medical equipment to our hospice patients. Agrace HME has two equipment warehouses, in Waunakee and Milton, where we use state-of-the-art software and systems to ensure medical equipment is delivered to patients quickly and accurately. In November 2024, Agrace Health, Inc. became the member of Agrace HME.
Program 3
$2.3MCommunity education - as the community's nonprofit hospice, agrace is dedicated to increasing awareness and understanding of matters related to serious illness, death, dying, care giving, and grief. With our guidance, our neighbors are better prepared to make decisions about the care they or their loved ones receive. In 2024, 180 nursing and medical students, residents and fellows, pharmacy students and students from other health care professions participated in rotations at agrace. Since the initiation of the agrace rotation program in 1995, approximately 3,283 students, interns, residents, fellows and working health care professionals from institutions of higher learning have completed an educational experience at agrace. Agrace receives no reimbursement for these rotations with the exception of an annual stipend from the uw-madison school of pharmacy.
Program 4
$1.7MAgrace Age at Home, LLC is a disregarded entity that was established in 2018 to help Dane County seniors live independently as they age. Services are provided in the client's home to support independence: personal care, housekeeping, cooking, errands, companionship, exercise support, medication reminders and much more. Agrace Age at Home serves both hospice and non-hospice clients. In 2024, Age at Home caregivers served 154 clients and provided more than 23,870 hours of in-home care. In November 2024, Agrace Health, Inc. became the member of Age at Home.
Program 5
$1.6MSpiritual & grief services - agrace's grief support program provides services to patients, family members and community members. In 2024, agrace provided grief services to 3,870 community members - both children and adults. The agrace grief support center in fitchburg, wis., hosts grief support groups and one-on-one support. In addition to individual counseling, agrace provides grief services through grief support groups, holiday remembrance programs, and the community traumatic death response team. These programs are offered for a nominal fee, and grief support groups are open to anyone in the community experiencing grief due to a death, not just to patients and families served by agrace. In 2024, agrace's community grief team also provided 21 educational presentations to 357 people. Audiences included university students, faith leaders, alcohol/drug treatment participants, senior center attendees, area therapists, community organizations and community members. Butterfly and brick memorials are available for individuals to memorialize or honor the life of a loved one. Although providing grief support is required by state and federal regulations, grief support services are not covered by insurance, medicare, or medicaid reimbursement. Agrace provides these services because the organization believes they are critical to helping families return to functional living after their loss.
Program 6
$1.1MAgrace Adult Day Center, LLC is a disregarded entity that was established in 2020. In July 2021, Agrace opened its doors to a freestanding Adult Day Center located at 1702 W. Beltline Highway, Madison. The Center is a safe, supportive place for older adults who need companionship and help with daily activities. As clients enroll at the Center, they have a functional assessment to help our staff understand their needs and interests so what they like can be woven into their daily activities. In 2024, the Center provided daytime care to 120 clients. In November 2024, Agrace Health, Inc. became the member of Agrace Adult Day Center.
