Program Reporting & Evaluation:In 2024, the AstraZeneca Foundation, its evaluation partners and its Creating Health Access for Next Generation (CHANGE) grant awardees created an evaluation framework that centers community voices and assesses improvements in healthcare access. As part of the framework, awardees submit impact reports and in return, receive evaluations from our evaluation partners that assess their progress toward goals and may include recommendations to address challenges. Awardees have regular access to Foundation staff and evaluation partners for assistance. By year-end, all six CHANGE awardees met or exceeded objectives overall on their evaluations and improved access to healthcare such as through expanding service offerings, addressing patient barriers, providing preventative care and screenings, integrating care coordination services and helping patients obtain health insurance and increase health literacy.

Community Advisory Board:As part of the AstraZeneca Foundation's Creating Health Access for Next Generation Equity (CHANGE) program, the Foundation and its evaluation partners virtually host monthly Community Advisory Board meetings to bring grant awardees and partners together for meaningful dialogue, shared learning and capacity building to help organizations maximize their impact. During this past year, these collaborative sessions focused on training and technical assistance to support grant awardee needs in reporting and evaluation and communications. Additionally, a learning forum brought together CHANGE awardees to foster connections and share lessons learned through presentations, interactive activities and group discussions.

Awareness Building:The AstraZeneca Foundation amplifies the work of grant awardees and the importance of community health commitments through communications like social media, its Annual Report, e-mails to its distribution list of 2,000+ individuals, videos and blogs. In 2024, the Foundation supported six longstanding grant awardees in sharing their impact through videography. To help CHANGE grant awardees build capacity, the Foundation developed and provided a communications toolkit, which offered press release and social media templates to share their award notice. The Foundation also participated in an AstraZeneca Health Expo with its CHANGE grant awardees to raise awareness for their work. Lastly, the Foundation submitted an abstract to the American Public Health Association Annual Meeting to share its Mentorship Program learnings and toolkit with other philanthropic and nonprofit organizations.

Grant Selection Process:The AstraZeneca Foundation's Grant Review Committee conducted a grant review to evaluate second-year Creating Health Access for Next Generation Equity (CHANGE) grant applications for organizations offering innovative, community-engaged approaches to improve access to quality healthcare. Each application was reviewed to ensure adherence to requirements, appropriate plans in place and a clearly defined budget. All grant applications were recommended by the Foundation Grant Review Committee for approval by the Board.
