Program 1
$717KRehousing in supportive environments ("rise"): has been in operation since 2019 with the goal of assisting domestic violence survivors achieve safety, security, and independence. We operate rise in collaboration with lifting up westchester and my sisters' place providing comprehensive wrap-around housing and support services to individuals and families impacted by domestic violence. This program focuses on stabilizing the survivor and family members in housing and providing robust case management, trauma- informed domestic violence services, and vocational and employment services. In 2024, we assisted 26 households for a total of 110 individuals to achieve safety and stability.
Program 2
$410KTurning point: this permanent supportive housing program provides housing and case management support services including home visits, advocacy, service coordination crisis intervention and life skills training. Our clients live independently in their own apartments, pay a portion of the rent from their income, and obtain support to assist them with remaining permanently housed. Program participants are formerly chronically homeless and live with disability. In 2024, we provided permanent housing and support services to 11 individuals. Tierra solida/solid ground is chhop's newest program developed to meet emerging client need. This preventative program for low-wage workers and/or immigrants provides temporary rental and/or utility arrears so families can remain stabily housed. This program is funded by the new york presbyterian community fund with support from the phelps healthcare foundation. In 2024 we conducted 40 intakes and provided funds to 17 households along with support service linakeg and referrals.
Program 3
$279KThe focus of fred's pantry is food equity. We offer "farm to pantry" food that is healthy and nutritious to community residents in an environment that respects the dignity of each individual. We also provide linkages to commuity-based sevices to food insecure community members to help address long-term sustainability. In 2024, we provided 195,624 healthy meals to 65,208 individuals, comprising more than 2,800 households.
