Program 1
$4.8MHunger programs:citysquare's food programs include the food pantry and child nutrition programs. The citysquare food pantry provides access to affordable and healthy food to dallas county residents. It also helps with chip, medicaid, snap, and tanf, three times a week. Neighbors can visit the citysquare food pantry every 15 days and choose the grocery items they want, eliminating the food waste that occurs with pre-made grocery packs. In 2024, the citysquare food pantry provided 10,327 neighbors with access to healthy, affordable food distributing over 1.9 million pounds of food. (continued on schedule o:)the food pantry is an entry point for citysquare's 16 other programs ranging from job training to mental health treatment. Additionally, citysquare operates a mobile pantry program that provides food to citysquare's permanent supportive housing programs through door dash delivery services. Citysquare's summer food service program (sfsp) and child and adult care food program (cacfp) provide meals year-round to children living in poverty in areas surrounding the dallas-ft. Worth metroplex. The major funder of these programs is the united states department of agriculture (usda); cacfp funding is used to provide children with after-school meals and sfsp funding is used to provide children with meals during the summer months.

$6.7MHope programs:neighbor support services provides individuals with case management and referrals to community resources either at citysquare or other organizations in the community. Long-term, goal oriented, intensive case management helps low income and homeless adults achieve key goals and move toward long-term self-sufficiency. About 438 individuals were served in 2024 leading to nearly 100 referrals to support services. Additionally, emergency goods such as bus passes and hygiene kits were distributed. (continued on schedule o) the homeless outreach team works to meet individuals where they are - whether it is in the streets or in encampments; in 2024, they engaged 201 individuals and placed 46 individuals into permanent housing while placing 58 on the housing priority list to await available housing. Americorps is a national service program dedicated to working in local communities to affect change through service. Citysquare operates the largest americorps program in north texas, and its members are a vital support to virtually every program that citysquare offers. In 2024, over 284 members worked over 114,231 hours to promote academic engagement, food security and social services across citysquare and other community organizations.

$208KCitysquare's health equity pillar utilizes the healthy people 2030 framework to drive our work to improve health and well-being. The two primary programming areas to execute this model are tailored to best meet the needs of the neighbor through our neighbor health support and neighbor resource center programs.the neighbor health support (nhs) program provides health and wellness navigation and support to citysquare neighbors in existing programs, such as supportive housing. It includes a community health nurse and community health workers to provide health education, navigation services and prevention screenings. In 2024, 57 neighbors were served leading to 1,352 patient navigation services with 83% having a doctor they go to for primary care. The neighbor resource center is dedicated to neighbors seeking information, referrals and navigation for urgent and essential needs. Resource navigators work alongside neighbors to empower them in navigating and accessing community resources or internal citysquare services. While some neighbors may touch both the nrc and nhs, others may only be served in a specific area. In 2024, 1,913 neighbors were served in the nrc leading to over 537 referrals with 64% successfully completing those referrals. In addition, 556 neighbors were assisted in obtaining health insurance.
