GLENN E CUSTIS IRREVOCABLE TRUST

The trust was established under an agreement to provide funds for the benefit of calvery baptist church located at 1920 west u.s highway 22, wilmington, oh (fein 31-1102425). Trustee can pay out 12% of the market value of the account as of 12/31. The church has the right to request that amount up to march 31 of the following year. The trust document does not require that it be for a specific purpose,however, the document does require the trustee to take into account all other sources of income before making any discretionary distributions.
