We're here to support you however and whenever you need, including connecting you to services that the parker jewish institute has long been a leader in providing.the hospice program provided 9,378 days of care in 2024 for 106 patients and their families.parker jewish institute for health care and rehabilitation serves as an investor and owner in a collaborative limited liability partnership, agewell new york llc through its disregarded entity, parkercare new york, llc, a single member limited liability company. Agewell new york llc operates a new york state medicaid managed long term care plan ("mltcp"), medicare advantage plan and a medicaid advantage plus plan, which promotes managed long term health care for a broad section of the community on a charitable basis. Agewell sold the mltc plan, effective october 1, 2022, to another managed care organization. As of december 31, 2023 agewell new york llc has shut down operations.
