Mission
To ensure that people with disabilities have complete access to the communities in which they choose to live. We provide information and referral, advocacy, peer support, independent living skills training and nursing home to community living transition.
Programs

The center provides transition services to individuals living in institutional settings wishing to transition to independent community living. We successfully transitioned four individuals to community living in the fiscal year, and have an additional ten individuals in the process of transitioning. We assist consumers in finding housing, household goods, and equipment needed to remain independent in the community.
