Social Services: Case Management Services: Delivered by general and IDA (Hurricane IDA) case managers, services address food insecurity, employment & training, court accompaniment for family law issues, translation/interpretation, juvenile justice, and applications for public services. Information & Referral Team: Assists clients with short-term needs and connects them to appropriate long-term service providers through targeted referrals and coordination. Senior Services: Culturally competent support for Latino seniors through case management, home visits, resource navigation, health access, and distribution of essential supplies including food baskets. Bilingual and Bicultural Senior Center: ACLAMO launched Montgomery Countys first Bilingual and Bicultural Senior Center - an inclusive, culturally affirming space where Latino elders can participate in social activities, health workshops, fitness classes, educational programs, & receive supporting services in Spanish. It fosters social connectedness & well-being for seniors who have lacked liguistically & culturally appropriate services. Housing Resources Center: Offers rental & utility assistance, housing advocacy, & conflict resolution, ensuring access to safe & stable housing for vulnerable families. Unemployment Support: Assists clients in navigating Pennsylvanias unemployment system, ensuring timely access to benefits and employment search tools.

Health and Wellness: Community Health Workers (CHWs): CHWs provide case management, education, and systems navigation to reduce health inequities, especially for uninsured and underinsured individuals. Benefits Acquisition Services: Provides bilingual, personalized assistance to individuals & families in applying for health insurance & other government-sponsored programs, including SNAP, CHIP, Medicaid, & utility assistance. This service helps clients overcome language, documentation, & system barriers to ensure timely access to critical benefits that support health, stability, & financial security. Health Literacy Education: Delivers preventive health education and chronic disease self-management training tailored to the linguistic & cultural needs of our communities. Immunization Team: Ensures community access to critical vaccines by coordinating bilingual outreach, education, and clinics in parnership with local health providers. The team focuses on overcoming cultural and logistical barriers, especially among underserved populations, to increase vaccination rates & protect public health. Promotores de Salud: Volunteer grassroots health educators who amplify public health messaging, increase community trust, and connect families to services. Maternal Mommy and Me Program: This culturally tailored maternal health intiative serves pregnant women & mothers with children under age two, providing bilingual education on prenatal care, child development, postpartum mental health, breastfeeding, and nutrition. The program also facilitates peer support, access to healthcare, and case management to ensure healthy outcomes for mothers and babies. Safe Haven Pantry & Closet: Provides emergency access to food, hygiene products, and basic household items for families in crisis, reinforcing dignity and stability.

Strategic Partnerships: Community Health Navigator Program (Jefferson Health Plans): Provides one-on-one support to Medicaid members with complex health needs, addressing care gaps & coordinating referrals for medical care, social services, housing, & behavioral health. Environmental Health Programs: Through weatherization, lead paint hazard mitigation, & healthy home education, ACLAMO reduces environmental risks that disproportionately impact low-income families, especially renters with young children. Advocacy and Systems Change: ACLAMO leverages its platform to advance equity in housing, education, language access, food security, womens rights, transportation, & immigration. Our work includes coalition-building, grassroots advocacy, & public policy engagement to achieve racial, social, & economic justice throughout the region.
