Behavioral health & clinical services is located at 2715 west 63rd street (773) 918-6100, 8741 s greenwood ave 2nd floor, 2524 s prairie 6th floor and 6033 s wentworth. We provide mental health assessment, treatment planning, individual, group and family therapy, case management, community support, mobile crisis intervention, and linkage and referral when external resources are needed. We served 3,811 clients this year.

Early head start / head start programs include at home support services based out of 8741 s greenwood ave, an agency head start location at 10534 s oglesby ave, and 13 head start early head start partner sites. We served 385 clients providing early intervention/ child development services.

Educational services talent search/early intervention college preparation and placement programs, 100 east 34th street, (312) 328-3543. We serve low income, first generation, 6th through 12th graders. We provide college preparation toutoring, summer school classes, cultural enrichment, act test preparation, and assistance with college admission and financial aid services. Program served 1,692 students this year. Residential services is comprised of thirteen locations. We provide housing and rehabilitation services for adults 16 and older who have disabilities. Employment training and monitoring is also provided. Between the integrated care facilities for developmental disabilities (icf/dd) and ten community integrated living arrangements (cilas), 107 clients were served. These are considered private residences and addresses and telephone numbers are not published.child welfare services: foster family care and adoption services operate out of two program sites. We serve wards of the state, their siblings, biological and foster parents through licensing of homes, placement, and permanency support, case management, post permanency support, and child protection. The program served 283 children this year.
