Case management - a formal system of assessment, care planning, and evaluation of services to assist qualified clients in realizing and maintaining their independence. Additionally, the program diverts individuals from nursing homes, allowing them to remain safely in their homes. Also, pre-admission screenings are performed to determine an individual's eligibility for admission to, or continued residence in, a nursing facility in our nine-county region.
