The alamo area community network is working to impact our community's social determinants of health by providing a cloud-based platform that allows non-profits, payers, and providers to conduct social service care coordination and electronic referrals on behalf of our clients/patients. In 2023, the aacn went through a major software transition to salesforce and launched a new tool called sync, which leverages salesforce's non-profit cloud product.
