Provide services under title iii of the older american citizens act of 1965. The ashland county council on aging, inc. (the agency) is a private, nonprofit organization established to aid persons 60 years of age and older in their efforts to live independently.

Title iii c1 - provides funding for congregate meal programs for persons over 59 years of age. Funds are used to purchase meals as well as support program operations. Title iii c2 - provides funding for home-delivered meals targeting frail and impaired persons over 59 years of age. Funds are used to purchase meals as well as support program operations.
Passport (pre-admission screening system providing options and resources today) provides families an attractive alternative to traditional nursing home placement by providing essential in-home services. Passport is a medicaid waiver program that provides screening, assessment, information and assistance, case management and in-home services to eligible low-income frail older adults.
