The Carolyn Adams Ticket for the Cure Patient Navigation Grant (AWARE) A program administered by the Illinois Department of Public Health,we assist individuals in navigating the breast cancer care continuum. The grant is part of the broader Carolyn Adams Ticket for the Cure initiative, which funds breast cancer awareness, education, and support services in Illinois. Community health workers work closely with our local hospitals, clinics and cancer resource centers and our team will work with coordinating communications and building rapport amongst clients and healthcare systems. We utilize our partnerships with healthcare providers, clinics, hospitals, and community organizations to facilitate access to healthcare services, coordinate care, and address social determinants of health. We collaborate with faith-based organizations, community leaders, and grassroots initiatives to enhance outreach, build trust, and leverage existing networks for community engagement.

THRIVE: Targeted Health Resources, Improved Visibility and Education for Maternal and Infant. This maternity program is a part of the Blue Cross Blue Shields Health initiaves program and assists clients in managing their pregnancy and preparing for childbirth. The program includes pregnancy risk factor identification, ongoing communication and monitoring, educational materials on pregnancy and infant care topics, a culturally appropriate speaker series, resources to support families in need, and a community baby shower. Clients are assigned Community Health Workers who assist in providing resources. Program benefits: Education: Information on pregnancy, infant care, nutrition, and healthy lifestyle choices. Risk Factor Education: Information on health disparities impacting Black Maternal Health. Monitoring: Ongoing communication and monitoring with program staff. Resources: Access to community resources and items to help alleviate financial strains. Thrive RIM
