Breaking The Barriers

Transitional housing program - this program provides temporary housing for approved individuals suffering from a mental health illness or drug and alcohol addiction. Population includes those with a criminal justice history as well as those moving from a restrictive inpatient or residential setting. We aide individuals in working with social determinants of health in a case management setting.

Cc mobile crisis services is a program that provides immediate support to individuals who are experiencing a mental health crisis or behavioral health emergency
