The center for comprehensive health practice (cchp), located in east harlem, ny, has a 63-year history of providing patients with comprehensive community healthcare and services in one location. Cchp relies on multidisciplinary, culturally competent approaches that address social determinants of health (sdoh) and barriers to care; inclusive space design that cultivates trust between patients and providers; and evidence-based, integrated programming to support the overall health and well-being of our patients and their families.currently, we provide an array of services spanning pediatrics to geriatrics, with targeted support for our patient population, including medication assisted substance use treatment outpatient substance use treatment group and individual counseling psychiatry parenting and health education overdose prevention training and naloxone distribution and primary care services, including internal medicine, hivprevention and treatment services, women's health, pediatrics

At cchp, primary care health services extends beyond a once a year physical with your primary care provider. Our team of physicians, care managers, patient navigators and counselors and nurses work together to address the medical, social, and emotional and child development needs of families. Cchp covers the spectrum from pediatrics to geriatrics and includes women's health, mental health counseling, psychiatry and prevention and/or treatment programs for hiv, hepatitis c, hypertension, diabetes, and asthma. Our emphasis on health education and early treatment helps our staff intervene before minor issues become lifethreatening. Our playrooms provide a space for early childhood development services, as well as enabling caregivers to have a safe place for their children during their appointments. During 2024, 2,019 patients were served with 10,441 visits.
