Touchstone - a short-term residential treatment collaboration with houses of hope providing 30-45 days of individual, group and family counseling; case management and psychiatric services for adults struggling with mental illness and addiction issues.

Case management - an additional component of the continuum of care; services provided by trained community support workers who work with women and men with both mental illness and substance use issues. Support is provided to individuals either in the individuals residence or the program office. Assistance is provided to identify day to day needs to reach independence in the community. As needs change, the services provided are adjusted to meet the individuals needs.

Expenses: $2.3M

Outpatient care - integrated, personalized care for adults and adolescents with co-occurring mental health diagnoses and substance use. Outpatient services include theraputic care in an office setting, medication prescription and support from licensed prescriber/provider; care management services; and peer support services.

Expenses: $1.1M

Houses of hope providing 30-45 days of individual, group, and family counseling; case management and psychiatric services for adults experiencing symptoms of mental health diagnoses and substance use issues.

Expenses: $848K

Day rehabilitation - long-term structured services to develop daily living skills for adults living with co-occurring mental health and substance disorders. This on-going program also provides transportation.

Youth transitions - transitional scattered-site housing and supportive living for transitional aged youth adults (ages 19-24).

Medication management - psychiatric evaluation and follow up, and prescription of behavioral health medications provided by psychiatrists and/or licensed aprns.

Partnership in empowerment and recovery (pier) - treatment, rehabilitation and support for women and men with severe mental illnesses, such as schizophrenia and bi-polar disorders. A collaboration with the community mental health centers and lutheran family services offering a full range of intensive services in home-based settings; reducing the need for hospitalizations; helping people live as independently as possible; and empowering people to enjoy a life not dominated by their illness.

Housing and supportive services - scattered site and project-based housing and supportive living for adults with mental illness and/or substance use issues and who have experienced homelessness or near homelessness.

Expenses: $464K

Certified community behavior health clinic (ccbhc) - centerpointe, inc. Was awarded two million dollars per year for two years to develop and operate as a ccbhc. The grant provides an opportunity to improve and expand services by integrating mental health and substance use care with physical health care, utilizing a wide variety of evidence-based practices, and promoting same-day access to high-quality, evidence-based care.

Centerpointe's primary care services are available to all ages. At centerpointe, we utilize evidence-based practices to include medication-assisted treatment. Our services focus on an integrated approach with mental health and substance use disorder care, to address an individual's overall wellbeing and provide the best quality care. Same-day access to behavioral healthcare is available, to include 24/7 crisis intervention services.

Linkpointe is a 24/7 civil protective custody program that centerpointe operates with the support of the lincoln police department.

Expenses: $325K

Squareone offers around the clock mental health services to anyone under the age of 19 and their parents/caregivers. Services include crisis response, crisis therapy and post-crisis ongoing support.
