Case management:served 67 hiv+ clients with 2,140 units of case management services to aid clients in accessing social and health services to optimize physical, mental and spiritual health.

Emergency financial assistance:emergency financial assistance was provided in the form of utility payments to help prevent homelessness and ensure good health for 15 families.

Food bank vouchers and meal program:624 services provided to 37 families to ensure proper nutrition to optimize good health outcomes. Services include: meals, vouchers to chattanooga area food bank, providing fresh fruits and vegetables, and supplying groceries from our food pantry.

Medical transportation assistance:transportation assistance is provided for doctor appointments, have labs drawn, and to access social services to assist clients in medical compliance. 315 transportation services were provided in the form of agency van, bus passes, and gas cards for 35 clients.
