Creating healthy schools and communities: program to help prevent obesity, type 2 diabetes, and other chronic diseases. This program addresses the ongoing need for sustainable policy, environment, and systems changes to support better physical and nutritional health. Chsc assisted six school districts to adopt local wellness policies and hosted two community conversations. Planning in progress for collaboration with local governments to create safer pedestrian and cycling routes through a partnership with go bike buffalo

Chq health collaborative: public health initiative (in response to covid-19 pandemic) to become a trusted source of accurate, local public health information and to bring about collaboration between health care providers and cbos. The program supported the creation of microcampaigns through a comprehensive media toolkit. Target groups include veterans, educators, food pantries, health services care coordinators, childcare workers, health care providers, move.chq physical activity promotion to general population, and school nurses and physicians.

Public health: strengthen cooperation or partnerships with local providers to improve key public health or health care outcomes. Participated in numerous meetings/conference calls with regional coalitions/board for covid repsonse. Enhanced our abilities to communicate with practices through contact management and mass email marketing platforms.

The purpose of this program is to expand public health capacity by supporting health care job development, training and placement in rural and tribal communities. The expected impact of this program is to enhance clinical and operational capacity in order to adequately address the population health needs of rural communities affected by COVID-19, including those dealing with the effects of long COVID1. The RPHWTN program addresses the ongoing critical need in health care facilities for trained public health professionals serving rural communities. This is done through the establishment of networks to develop formal training/certification programs
