Iowa community hub was one of 20 organizations nationwide awarded capacity-building funding by the usaging centers of excellence to strengthen community care hubs (cchs) in intergrating health and social care. To ensure equitable access to area agencies on aging programs and services, the hub is creating referral pathways to arthritis-appropriate, evidence-based interventions including falls prevention, and establishing data governance to enhance interoperability and secure data sharing between the hub and all six aaas.

The prairie meadows betterment grant has enabled the iowa community hub to expand the community food project, tackling food insecurity and reduce chronic disease among low-income iowans. In 2024, in partnership wih capital city fruit, the hub provided free monthly home deliveries of fresh produce to approximately 325 at-risk individuals enrolled in community- based health programs. This funding has significantly expanded our reach, ensuring more iowans have access to nutritious food and essential support.
