The next level of care at eastmont is assisted living for those individuals who may need assistance with activities of daily living. The seasons is licensed as assisted living and is located on the eastmont campus. The seasons provides 41 one-bedroom apartments and uses an activity-driven social model with an emphasis on fellowship, enrichment, and educational opportunities. Residents have access to licensed professional nursing staff on campus 24 hours per day. Individuals have tailored programs that can include assistance with activities of daily living, medication management, health maintenance activities, transfers or ambulation, oxygen, and additional companion or home health needs.
Eastmont also provides a health center, which is licensed for medicare and provides both short-term rehabilitation in 10 rooms and long-term care in 11 rooms. They are designated as semi-private (can accommodate two occupants), but in response to the market, eastmont operates them as private rooms. The health center provides skilled nursing, and physical, occupational, and speech therapy in an interdisciplinary environment. Additionally, spiritual guidance, social work, case management, and care services are provided for the overall psychosocial well-being of the patients. The care center is guided by a medical director, licensed nursing home administrator, and director of nursing.
