Transformation works, a collaboration with local law enforcement agencies that provided case management services to gang involved youth and youth at high-risk for gang involvement (ages 12-24). Participants received one-to-one interventions and worked with their case manager to set individual goals, maximize personal strengths, and address barriers to success. In addition, family members were provided support, resources and referrals for services. Youths were provided intensive case management services to help deter them away from high risk gang activity.
