IN DECEMBER 2019, WE ESTABLISHED A NEW DIABETES COORDINATED CARE PROGRAM FOR PATIENTS WITH UNCONTROLLED DIABETES AND OTHER METABOLIC SYNDROME ISSUES. OUR PROVIDERS AND FRONT-LINE MEDICAL STAFF IDENTIFY AND RECRUIT PARTICIPANTS WHO ARE A GOOD FIT FOR DIABETES GROUP VISITS. THE MONTHLY GROUP VISITS INCLUDE 2-3 STAFF MEMBERS OVERSEEING 10-15 PATIENTS AND THEIR FAMILY MEMBERS. THE INCORPORATION OF FAMILY MEMBERS AND/OR OTHER SUPPORTING FRIENDS IS A VALUABLE ADDITION TO OUR DIABETIC CARE.
