Hcpf haas:this program supports the care coordination of the medicaid population for the state medicaid agency. Haas is an acronym that stands for health information exchange as a service. Under this program we provide the state medicaid agency and the regional accountable entities (raes) access to admit discharge and transfer data(adt) to improve care coordination for their beneficiaries. This programs also provides technical connectivity dollars that cover the implementation fees for medicaid eligible providers to connect to many of corhio's hie products.

Hcpf ecqm:corhio provides support for the electronic clinical quality measure (ecqm) program managed by the colorado department of health care policy and financing (hcpf). This program focuses on providing insight into quality metrics across several participants within the corhio community. Corhio works with practices to collect ecqms and report them to the state medicaid agency, the department of health care policy and finance. They in turn, report the numbers to the center for medicaid and medicare services (cms).

Oehi swim:the office of ehealth innovation (oehi) statewide identity management (swim) program provides ongoing identity management services to the state of colorado to promote effective patient matching services for health agencies.

Obh jails:this program provides access to the longitudinal health record (pc360), for medical professionals providing care to individuals who are currently in jail facilities. This access offers both the overall health record as well as access to medication history to provide the most effective transition care. Additionally, we have built connections to several county jails to have them send data out of the jail and into the hie upon inmate release.
