Community Hero Action Group

An initiative that allows our org to swiftly respond to emerging community needs by leveraging partnerships to launch targeted short to long term initiatives that address disparities and promote equity which have included: Opioid Awareness Initiative that provided community education resources and addiction recovery support Food Insecurity -A partnership collaboration that combined the importance of providing nutritious food to underserved communities and sourcing from Black farmers. Organ Donation Awareness - A campaign to Life to increase organ donation among African Americans through awareness and education Vaccine Education & Administration - Provides up-to-date vaccine resources and immunization education and also vaccine clinics.1920 Attendees 330 Vaccines administered

The Community Advocates for Senior Empowerment CASE Program is an evidence-based initiative designed to provide education on depression and social isolation among seniors while connecting participants with relevant community resources. The programs launch and preparation began in Q4 focusing on organizational training certification and initial outreach planning to ensure effective implementation. A secondary component of the program involved establishing a tracking system and awareness campaign to enhance engagement and impact measurement. Additionally the program incorporated requirements for applicants of the EXCEL Program an initiative aimed at increasing cultural diversity in the mental health profession. EXCEL provides financial support for certification exam fees licensing exam fees and a one-year membership in a professional organization with a mentorship component helping aspiring mental health professionals navigate their career pathways.

Continued to develop and produce a live radio show that provides health and wellness information for women. Local and national guests appeared on the show including health professionals academic researchers clinical researchers community leaders and local and state officials. Provided essential health discussions featuring on topics like breast cancer disparities pay equity and mental health. Estimated listening data: 32,390 average number of person listening for at least five minutes during a 15-minute period

The week-long hybrid conference featured leading health professionals researchers and community advocates discussing pressing health equity issues. Focus areas included mental health maternal health chronic disease prevention and access to culturally competent care. It also included panel discussions keynote presentations and interactive Q&A sessions ensuring that over 500 participants engaged with health experts in meaningful ways. Our org partnered with statewide and regional health organizations enhancing awareness of available health resources.

Non-partisan voting registration and mobilization community activity and events; civic education

The organization served as a fiscal sponsor for two 2 projects that align with our mission but dont have their own 501c3 status. Through these fiscal sponsorship arrangements we provided administrative support financial oversight and tax-exempt status to projects focused on addressing local disparity issues with a special focus on civics and also education of health-related community resources available and ongoing multi-county community education forums. We processed $49,400 in grants for these projects.
