HIV/HCV TESTING EDUCATION & TESTING We provided HIV/HCV screening, linkages to care and referrals to prep. We collaborated with Yale and The City of Hartford to assist participants with the necessary services they may need.
HOMELESS & MOBILE OUTREACH Our Homeless outreach engage with participants by means of housing focused case management which gets them documentation ready and connect them to the necessary resources for hosuing, food and other benefits. Our mobile outreach provides a roving, on-demand service to our participants in Greater Hartford and other surrounding areas. A nurse is available for testing, screenings and wound care.
