Coordinating Center For Home And Community Care Inc

Community First Programs (CFP), under contract with the Maryland Department of Health, The Coordinating Center provides care coordination services for individuals living with disabilities and/or complex medical and/or behavioral needs, who are enrolled in one of more of Maryland Medicaids home and community-based programs; serving on average over 3,000 people a year.

The Coordinating Center partners with the Maryland Developmental Disabilities Administration (DDA) to provide coordination of community services for people with intellectual and developmental disabilities and their families.

Other programs including service coordination and care management services for children with Autism, formerly homeless adults and children and adults with complex medical needs and disabilities. Programs include the Marylands Home and Community Based Services Waiver for Children with Autism Spectrum Disorder, Montgomery Countys Housing Initiative Program, the Model Waiver Program, Maryland Developmental Disabilities Administrations (DDA) Coordination of Community Services Program, VIPhysicians&Kids medical home program, Life Care Planning and Case Management services provided by the Medical Legal Services Division and the CARES Program for PPMCO members.
