Idaho health neighborhood clinic - a primary and urgent care in nampa, id that provides same day access to behavioral health and clinical pharmacy services that are culturally responsive and community-rooted, while developing a diverse healthcare workforce through training and employment pathways for international medical graduates and underserved future professionals.
Moshi -mobile opioid solution for whole healthcare integrationthe moshi project aims to address key issues in the effective identification, triage, management, and follow-up of high-risk opioid patients seen in the service areas in rural idaho and oregon through the implementation of mobile detox/induction, tele-mat, and tele-sud/oud treatment. Cornerstone is developing a hub and spoke model to provide oud/sud services through telehealth and mobile outreach and training center that incorporates dental, pharmacy, oud/sud, behavioral health, primary care, and rural emergency departments.
Cornerstone whole healthcare organization provided rural outreach services and programs in accordance with the established mission and purpose of the organization.
Gritte - the greater rural idaho telehealth team expansion (gritte) project aims to expand access to critical behavioral health services and treatment resources in rural under-served communities across idaho. Over the next 5 years, the gritte project will work to develop a sustainable direct to consumer (dtc) service network and tele-behavioral service delivery model to reduce the barriers to accessing behavioral health in your community and catchment area.
