Department of health and human services grant - serves 18 counties in south central north dakota. Served 526 consumers in direct service, this includes the south west counties in north dakota. Provided 2,405 information and referrals for individuals. Assisted 9 individuals with enrollment/comparison in medicare part d plans. Made several trips to the standing rock sioux reservation providing money management classes and drivers permit classes to students. Assisted consumers with tax returns and with the north dakota rental rebate. Provided technical assistance and/or advocate for accessible construction and renovation of businesses. Provided demonstrations on assistive technology in various counties.

State grant - same achievements as in 4(a) above. The service area for this grant consists of counties in south west north dakota.
