Adults: the organization provides community based psychiatric rehabilitation services to seriously and persistently mentally ill adults. This includes individuals with schizophrenia, major depression, bipolar, panic, anxiety, post-traumatic stress disorder (ptsd), borderline personality disorder, etc. Our services include case management, skill-based training, resource connection, educational and social activities, and psychosocial rehabilitation (psr) in collaboration with outpatient psychiatry. We provide supported employment services to assist persons with a mental illness or developmental disability in joining the workforce which includes matching the individual to competitive employment opportunities, job coaching and support. Youth: the organization provides services to youth which include case management, skill-based training, resource connection, educational and social activities, and psychosocial rehabilitation (psr) in collaboration with outpatient psychiatry. We also provide school-based therapy and family support to children with serious emotional disturbances and their families. This includes children with schizophrenia, major depression, bipolar, panic, anxiety, post-traumatic stress disorder (ptsd) among many other conditions. Both adults and youth: we provide services under a healthcare home model for individuals on medicaid that need monitoring of their medical and behavioral status. The services are to improve the individuals health status and reduce hospital admissions. We served an annual average of 281 adults and youths during 2024.
The organization operates psychiatric group homes supporting up to 24 individuals with complex and co-occurring conditions who require significant support to be successful in the community. These homes are staffed 24 hours per day, 365 days per year. The goal is to successfully integrate persons with mental illness back into the community. In pursuit of that during 2024 we moved 3 persons to our clustered apartment program who can now live semi-independently.
We provide 24 hour a day crisis intervention services for adults, youth and families. During 2024, we provided 1,695 hotline crisis calls and interventions either in the office or in the community, emergency room enhancement care coordination, assisting persons to identify affordable housing solutions, and community behaviaoral health liaisons to assist with justice-involved persons needing behavioral health intervention.
