Crisis navigation - the extra mile works in conjunction to provide crisis services to those with substance use disorders(sud) in the clhsd catchment area. The program works to bridge the gap between individuals seeking assistance with sud and the resources that are available to help them. Through the crisis navigation program, adults with sud or co-occurring disorders, who are experieincing an addiction-related crisis can conenct with a peer navigator, who will help ameliorate the experience and navigate the challenges of recovery, including connections to behavioral health services and community resources.
Care coordinator - the purpose is to provide direct and indirect services to persons with mental and behavioral health disorders in an effort to help individuals remain in a community setting and reduce the risk of hospitalization, homelessness, physical health problems, legal problems, or arrest and incarceration while promoting recovery and resilience
