Hypertension:hypertension is a complex medical issue requiring close monitoring. In 2024, we managed patient care for approximately 100 patients who have hypertension. We provide needed medications (quarterly) and bloodwork (annually and as needed) for these patients.

Diabetes:diabetes is a complex medical issue requiring close monitoring. In 2024, we managed patient care for approximately 30 patients who have diabetes. Most of these patients also have hypertension as a co-morbidity. We provide medications, diabetic testing supplies, and bloodwork on a quarterly basis for these patients.
