Community Health Home (aka Housing and Community Services) is comprised of three essential programs: the Oak Street Community Cabins (OCC), the Community Mediator program and Stop The Hate project. Oak Street Community Cabins (OCC), located in Oakland Chinatown, is a 24/7 emergency shelter specifically for women experiencing homelessness. Over the past year, OCC has provided emergency housing for 45 women and women-identifying individuals, ages 20 to 65, served over 10,682 meals, and successfully housed 27 participants. Our Community Mediator program, a non-emergency response initiative in Oakland's Chinatown and Downtown, focuses on community safety by building relationships, de-escalating situations, and connecting individuals with necessary resources. They defused 65 tense situations, provided 9,838 outreach and intervention services to houseless individuals, and offered 722 referrals to community resources. Before transitioning in February 2024, we also removed 144,690 pounds of trash, power washed 484 blocks and eliminated approximately 19,375 square feet of graffiti. As part of the statewide Stop The Hate campaign, this program aims to raise awareness of bias, educate the community, and foster understanding to combat hate and discrimination. Over the past year, our activities included two social media campaigns, tabling at five community events, partnering with three schools, and organizing 7 community gatherings, empathy tours, and conferences.
