Mission

To bridge the gap between hope and opportunity by educating and enriching the lives of those affected by organ tranplant.

Programs

1 program

Program Development: In 2024, Georgia Transplant Foundation focused on strengthening its core programs while making targeted adjustments to ensure long-term sustainability. Two underutilized programs were phased out to reallocate resources toward high-impact services, following a careful review process and social worker feedback. GTF enhanced its Evaluation Assistance and Dental Assistance programs to better serve pre- and post-transplant patients and expanded awareness of its services outside metro Atlanta through targeted outreach to dialysis centers, clinics, and transplant social workers statewide. Additionally, the organization laid important groundwork for long-term outreach growth, including refining patient data collection protocols, and increasing touchpoints within African American communities. These efforts supported GTF's mission to serve as a bridge between hope and opportunity for Georgia's transplant community.
