Health home:provides care coordination for high-cost, high-need medicaid recipients with multiple chronic conditions, whereby all of the professionals involved in a member's care communicate with one another so that all needs (medical, behavioral, health and social service) are addressed in a comprehensive manner.

Prevention:provides evidence-based hiv,hepatitis and sti prevention interventions, patient navigation services, education and access to prep, comprehensive risk-reduction counseling, confidential free hiv, sti, hepatitis b and c testing, harm reduction services for people who use drugs and alcohol, including overdose prevention services,, syringe exchange services and drug use education.
