Driven by commitments to health, saefty and compassion, representatives from fifteen agencies comprise the interdisciplinary steering committee.

Complex care-complex care coordination-hips role is to lead sustainable systems change to advance care coordination for our most vulnerable residents. Hip convenes partners to tackle the challenges of complex care coordination across multiple care settings including the hospital, clinics, mental health and substance use disorder treatment and housing agencies.
