Primary preventive healthcare system - healthy archuleta is supporting the primary preventive healthcare system through community-initiated solutions such as: 1) developing and facilitating a community-based health fair to connect community members (n=328) with low-cost laboratory tests (n=228), screening services (n=129), healthcare, mental health, and oral health care providers, and community resources (n=28); 2) working with the four largest primary preventive healthcare organizations to form an archuleta rural health network focused on increasing care for uninsured, underinsured, and medicaid eligible community members; 3) expanding access to mental and behavioral healthcare for underserved populations, 4) planning and implementing a community participatory nutrition security|health equity assessment and community survey (that has engaged over 600 community memebrs) to understand the community's current state of primary healthcare access and utilization and structural barriers impacting community members; and 5) engaging partner organizations (n=35) in the assessment process and the health partner coalition. Additionally, healthy archuleta provides prevention-focused education related to community health and wellness and identifies and supports policy, systems, and environment (pse) changes and/or programming.
Community cohesion - healthy archuleta works to advance community cohesion across affinity groups, including youth, those sharing similar cultures, growers and producers, and other sessions related to a healthy local food system and health and wellness. In 2024, over 15 youth offered services to the community through events such as food drives, food summit, and health fair. Additionally, over 65 community members participated in at least one affinity group session. Other activities hosted and/or facilitated by healthy archuleta related to community cohesion in 2024 included sharing information at community events (n=6), food demos (n=2), supporting community gardens (n=2), developing partnerships with institutions of higher education, engaging volunteers (n=166) and enhancing awareness related to the nutrition security and health equity via facebook, e-news, instagram, and local news outlets (n=2,300).
