Our mission is to foster a healthier, more inclusive and connected portland for adults 55+ through educational and recreational programming, support services, and care.
Programs

Aging, disability and veteran servicesas the lead agency of the n/ne consortium in collaboration with the urban league of portland, cfpa contracts with the multnomah county aging, disability, and veterans services division. We offer assessment and care coordination for older adults in northeast/north portland, assisting over 350 clients in maintaining their independence at home. During the same period, we provided 2,191 care coordination hours to clients and caregivers, responding to 1,644 requests for information and assistance. Our transportation services improved access to essential resources for more than 220 seniors through monthly bus passes, tickets, and coordinated rides for medical appointments and community activities.
Enhanced resident services & permanent supportive housingour enhanced resident services program supports about 445 residents of low-income and affordable housing communities by providing direct assistance, resource referrals, and community-building activities that enhance the quality of life for both individuals and their residential communities. Through our permanent supportive housing contract, we offer voluntary on-site case management, peer support services, and community activities for those transitioning from homelessness to permanent housing.
