St. John's United Hospice quickly realized that our patients live a better quality of life and often live longer because of hospice care. We feel the act of transitioning from this life is one to be lived beautifully and as best as possible. In fact, living those last few months, weeks, and days with the best life has to offer is all we could hope forfor ourselves and for our loved ones. And that is exactly what St. John's does for our hospice patients. Continued on Schedule OIn 2024, St. John's hospice team provided 15,138 days of end-of-life care for 159 individuals through individual visits by registered nurses and home health aides. Social workers work closely with the families and their loved ones, helping them through difficult decisions and navigating healthcare options. SJU Chaplains provide spiritual support and visits to both patients and families. Volunteers, a required component of hospice care, visit patients regularly and are often with patients in their final moments. Our social workers and chaplains continue their good work after the patient is gone, working with family members and friends through the grieving process.
