Local programs: local & statewide programs (il)hcci provides local and state-wide practice providers with training, curriculum and analytics tools to improve their knowledge and skills in the field of home-based care to equip them to serve more of the older adults who are homebound, home-limited or living with serious illness that cannot currently access home-based primary care due to workforce shortages.

Data analyticsresearch: to solidify its place as a unique model of care, it is critical that home-based primary care (hbpc) be built on evidence-based practices. Hcci is continuing to gather insights and develop a greater understanding of best clinical and operational practices. In addition, hcci works with its partners to provide ongoing research to demonstrate the benefits of hbpc and best ways to implement and expand practices.
