The center was incorporated under the laws of the state of colorado in 1975 for the purpose of providing a community center board to coordinate programs through inter-agency cooperation and local agencies to provide services to persons with developmental disabilities in grand, jackson, moffat, rio blanco, and routt counties.

Case management - determines eligibility for services and supports, service and support coordination, and the monitoring of all services and supports delivered pursuant to the individualized plan, and the evaluation of the results. Served 335 individuals.
Adult supported living services - provides individualized living services for persons who are responsible for their own living arrangements in the community. Served 21 individuals.
Early intervention services - are supports for children from birth through age two which offer infants and toddlers and their families services and supports to enhance child development in the areas of cognition, speech, communication, physical, motor, vision, hearing, social-emotional development, and self-help skills; parent- child or family interaction; and early identification, screening and assessment services. Served 95 individuals. Expenses: 364,939 revenue: 521,949 family support. Served 120 individuals. Expenses: 139,798 revenue: 115,547 childrens extensive support. Served 2 individuals.
