Mission
To provide education, therapy, social, residential and other support services to people with developmental and other physical disabilities.
Programs
3 programs
Our state certified day habilitation programs provide adult basic education, pre-vocational exploration, nursing services, social and recreational opportunities and specialized transportation to adults with multiple physical and intellectual disabilities. Services are provided in attractive, barrier-free environments in both monmouth and ocean counties. Pre-vocational exploration includes the areas of food service and childcare. Evaluation and training in the use of assistive technology is integrated into the programs. Physical therapy, occupational therapy, and speech language therapy may be provided as components of a transdisciplinary approach to service delivery. Additional funding for this program is provided through federal grants.
Residential options consist of 10 locations, 7 group homes, 2 supported/supervised apartments and 1 independent living apartment complex. Located in monmouth and ocean counties. These barrier free, accessible locations provide 24/7 residential care to 89 adults with multiple physical, intellectual and developmental disabilities. Personal assistant services are provided to the tenants in their residence. A wheelchair equipped vehicle is also available at each residence to assist tenants with attending day programs, community integration activities, and medical appointments. These programs are subsidized by funds available from hud and nj dhs.
The early intervention program (ei) provides early identification, evaluation, and comprehensive services in monmouth and ocean counties ages birth to three years old. The targeted evaluation team (tet) provides assessment for eligibility into the ei program in monmouth county. Once the infant or toddler is evaluated and found eligible to receive services in the ei program, an individualized family service plan (ifsp) is provided, and services will begin in the child's natural environment. Agency team members (speech therapists, physical therapists, occupational therapists and developmental interventionists) assist families in acquiring skills, knowledge and strategies to manage the child's disabilities or delays utilizing the "engage and exchange" model with our parent partners, enhancing each caregiver's ability to meet their child's social emotional and developmental needs. In fiscal year 23/24, the agency provided an average of 605 services for families per month and provided eligibility evaluations to an average of 127 children per month in monmouth county. Our targeted evaluation team provided annual evaluations (average 58 per month), exit evaluations (average 44 per month), additional assessments (average 50 per month) and ifsp services (average 125 per month).
