Albemarle charlottesville regional jail - jail re-entry navigation supports inmates with diagnosed mental illness, plans for their release, and facilitates their transition to the community by making connections for mental health or substance use treatment, benefits, housing, and such. In 2024, jail re-entry worked with than 50 persons.

Albemarle county's hart team - hart is a co-responder team comprised of members of the human services, police, and fire & rescue departments plus a pmh community navigator that provide follow-on visits to 911 callers experiencing mental health trauma or crises in mental health or substance use.
