Care coordination programs: health homes: provides a wide-variety of support and services in in non-institutional settings for adults and children with mental health and substance abuse challenges utilizing a person-centered approach to ensure whole person wellness. Services also include assistance with travel, financial entitlements, and educational and vocational needs. Support for at risk individuals to help navigate the mental health system and link them to necessary services and support in the areas of health, financial, entitlements, educational and vocational needs. Medicaid service coordination/crisis respite: outreach and referrals for adults and children who have challenges accessing resources, benefits, support services, housing, and educational opportunities. Care coordination continually provided services to over 500 individuals.

Personalized recovery oriented services (pros): program description: the mhanc pros program provides comprehensive, recovery-oriented services to adults with serious mental illness. It integrates clinical treatment, rehabilitation, and support services to help participants achieve personal goals related to mental wellness, medication management, employment, education, housing, and social relationships. Pros target population is adults diagnosed with serious mental illness residing in nassau county, ny, who require structured support to improve functioning and quality of life. Services are provided via individual and group therapy, medication management and psychiatric services, vocational rehabilitation and job readiness training, wellness self-management and peer support, crisis intervention and care coordination.program outcomes: served approximately 117 individuals during the tax year. Program participants achieved employment or educational milestones, reduction in psychiatric hospitalizations among enrolled individuals, increased community integration and independent living skills. This program directly supports mhanc's mission to promote mental health recovery and community inclusion through person-centered services.core program:the core (community outreach, recovery, and empowerment) program provides mental health support, advocacy, and recovery services to underserved populations in nassau county. The core program provides comprehensive mental health services, peer support, and community outreach to individuals and families in nassau county affected by mental illness. The core program provides weekly individual counseling sessions with peers and clinical counselors, and skill development for members of the community with serious mental illness.

Other mha programs: financial management services: this program assists individuals in managing their personal finances to ensure housing stability, prevent homelessness, and promote responsible use of benefits. Services include budgeting assistance, financial-literacy education, and individualized money-management support. In 2025, the program served 532 individuals.veteran's health alliance of long island (vhali) & vet2vet:these initiatives provide outreach, support, advocacy, and training to veterans and military families. Services include peer support, linkage to clinical and community resources, training, and community education. In 2024, vhali/vet2vet supported approximately 5,300 veterans.advocacy, education & training:in partnership with the nassau county department of mental health, chemical dependency & developmental disabilities, this program provides community-based psychoeducational training for behavioral health providers, peers, and community stakeholders. Trainings focus on strengthening the behavioral-health workforce, enhancing community knowledge, and promoting mental-health awareness. In 2024, approximately 2,572 individuals received training.consumer link peer-run division:consumer link delivers peer support, outreach, and engagement across mhanc programs and throughout the community. Peer specialists provide lived-experience-driven support to promote recovery, empowerment, and community stability. In 2024, consumer link provided support to 11,234 individuals.peer respite programs (turquoise house & rose house partnership):mhanc's peer-run crisis respite programs offer short-term, community-based alternatives to hospitalization for individuals experiencing mental-health crises. These programs provide stabilization, crisis navigation, and recovery-oriented support in a non-clinical setting. In 2024, more than 225 individuals were diverted from psychiatric hospitalization through peer respite services.
