Housing voucher program: mhr administers housing vouchers in the 7-county metro area for individuals living with mental illness, chemical dependency and other disabilities. These rental assistance subsidies are from a variety of sources including hud shp (ramsey, anoka, dakota, and hennepin counties), bridges, section 8, housing trust fund and ramsey supportive housing voucher program. In 2024, 1,425 clients were served.

Assertive community treatment (act) is a service model provided by community- based, mobile mental health treatment teams. The act team approach is designed to provide comprehensive psychiatric treatment, rehabilitation, and support to persons with serious and persistent mental illness, or personality disorders with severe functional impairments, to live independently in the community. Persons served by act often have co- existing problems such as homelessness, substance abuse, frequent hospitalization, and/or involvement with the judicial system. People typically served by act teams often have needs that have not been effectively addressed by traditional, non-intensive services. Team members work with people in their homes, work setting, or places in the community where additional support might be needed. In 2023, 203 clients were served.

In 2024, mhr served 13,923 individuals. While the majority of our clients reside in hennepin, ramsey and dakota counties, we provide services in 24 counties throughout minnesota. The vast majority of mhr clients are low- income or living below the federal poverty level. 96% of our clients are enrolled in public programs; 3% are uninsured and covered through county contracts; and 1% are enrolled with a commercial insurance plan (including medicare commercial plans). Our current programs and services include: "targeted case management (tcm) teams (adult, transitional age youth and children's tcm): in 2024, 2,561 clients were served. "assertive community treatment (act) teams: in 2024, 203 clients were served. "metro intensive treatment team: in 2024, 429 clients were served. "seward community support program: in 2024, 232 clients were served. "special needs basic care (snbc): in 2024, 9,113 clients were served. "housing voucher program: in 2024, 1,425 clients were served. "outpatient substance use disorder program and medica sud program: in 2024, 128 clients were served. Additionally, in 2024 mhr provided the following support services for clients. O mental health outreach clinic (mhoc): the clinic is an essential community provider that serves high-risk, special needs, and underserved individuals. The clinic specializes in providing in-home and telehealth therapy to individuals and families regardless of their financial limitations. O mhr health & wellness: our health and wellness department helps to provide training, skill development and resources to our mhr programs so that they can assist in meeting the health needs of the people we serve. Some of the current programming includes tobacco cessation initiative, healthy eating initiative, and a comprehensive dental program to connect clients to best practices in dental care. Mhr conducts an annual statisfaction survey. In 2024, 96% of survey participants indicated that they were very satisfied or satisfied with the services they have received from mhr. 96% of survey participants indicated they would recommend mhr to a friend or family member and 91% of survey participants indicated their life is better since receiving services from mhr.
