To improve the health of low-income people who face the greatest barriers to care access.

Upcounty: the Upcounty Clinic in Germantown serves over 30% of the organization's total caseload. This six-exam room clinic, open five days per week, provides a full array of primary care services, including behavioral health to those in need. Activities also include regularly scheduled clinic sessions focused on the Asian immigrant community. As with the other clinic sites, patient appointments are both in-person and via telehealth.

Head Start: The Organization contracts with the Montgomery County Department of Health and Human Services to provide health management services to children three to five years old who are enrolled in the Head Start and Pre-Kindergarten programs. Case management services and health screenings are offered to all Head Start/Pre-K program children and their families. In the process of assessing the needs of children, parents in need of primary care may be identified and referred to the Organization, if they have no insurance. During the school year, the Organization provided services to children in about 80 classes in 35 Montgomery County Public Schools and community-based centers.
