Providing social engagement opportunities for seniors and caregivers.our program has increasingly focused on reaching solo seniors and isolated caregivers for social engagement opportunities. We build awareness and understanding of the emerging population of solo older adults in (service area) and build organizational capacity to support solos with decisions around health and well- being. Our program: (1) established monthly solo senior resource groups in multiple community locations (e.g. Hallie q. Brown center, st. Paul recreation centers); (2) engages 6-10 education experts/speakers to lead discussions on issues including estate planning, mental health, physical health, and more; (3) provides social engagement opportunities for solo seniors and isolated caregivers in accessible locations; (4) provide back-up planning tools and support.

Providing health and safety support programs for seniors and caregivers. We provide in-home safety checks and coordinates minor home modifications and repairs as needed to address safety issues. Volunteers provide help with seasonal chores to help to maintain the home and reduce the risk of falls due to ice or attempting difficult chores. Our outreach nurse provides vital safety and care plan strategies and serves as a resource for community programs and seniors served by them. To provide regular, comprehensive education opportunities for seniors, caregivers, and members of the public covering a broad range of topics to include, but are not limited to: decluttering and safety at home; vision loss support resources; stress relief/self-care; and medicare/insurance navigation. Our education programs helps to round out any service plans developed with seniors, caregivers, and our staff, providing additional information and resources.
