Nm heart disease & stroke initiative: the heart disease and stroke funds from the department of health funds two primary initiatives focused on cardiac care and diabetes prevention. One project is to fund the million hearts initiative's priorities are to optimize care, keep people healthy, and improve health outcomes. In addition, this grant funds the work of the native american partnership for health and wellness prevention program (nap). The mission of nap is to facilitate a diverse partnership to build capacity through leadership, advocacy, and collaborative learning. Nap is lead by a core committee, a group of volunteers who provide strategic direction and leadership for our organization's tribal community engagement, training opportunities, and annual conference.

Phrma grant: phrma works to drive patient-centered progress in innovation, affordability and access to life saving prescription medicines. This grant supports the workgroups of the chronic disease prevention council: heart disease and stroke; prevent diabetes; and health equity. For the heart disease and stroke workgroup. Funds specifically provide support for printing and distribution of a fact sheet that is included in food bank boxes covering diabetes care related to covid-19 protocols. For the prevent diabetes workgroup, funds are used for creating and promoting a two-webinar series called "food as medicine" to let patients know about the national diabetes prevention program and the diabetes self-management education series. For the health equity workgroup, funds ware used for project outcomes for the advocacy & action; education; and collaboration committees, including materials and event support with stipends and community outreach.

Peer support worker project grant: cdpc is proposing, with continued support from bhsd, to further develop and sustain the new mexico peer support network assisting certified peer support workers and provider agencies to improve the outpatient behavioral health outcomes of clients. Cdpc worked with numerous peer support stakeholders from july 2022 to june 2023 to determine key priority areas along with clear goals and actions to create and sustain a peer support network and strengthen the practices of agencies utilizing certified peer support workers. Cdpc is applying for a two-year extension of the original pilot project to prioritize and operationalize the set of preliminary goals identified by certified peer support workers (cpsw) subject matters experts and practitioners from across the state.
