Management of hazelwood facility - major accomplishments include:- end of september 2024: heat boosters installed on the dishwasher to help maintain regulatory temperatures.- october 2024 - december 2024: - a new gas stove was installed in the kitchen to replace the old non-working one in october 2024.- the nursing department did a mesa sueca (cuban holiday meal), prepared by the nursing staff. They also did a soup & stew day where all shifts could partake of homemade soups. This was a part of their ongoing program to retain nurses and decrease/eliminate higher turnover as they had in the past. Retained nurses get to know their patients and facility processes better and provide a higher quality of care for our patients.- the nursing department lost only 1 nurse during this timeframe, and it was due to her achieving higher education as an aprn and leaving for an aprn job.- hazelwood's enrichment department (ec) partnered with the louisville ballet company for a creative movement exercise.- some residents got to see the louisville ballet production of the nutcracker, the kosair shrine glow circus and/or christmas at the kingdom kentucky kingdom, free of charge.- hazelwood ec- halloween party (clients assisted in making some costumes) and christmas party/activities (made ornaments, with additional activities for the month, and festive spirit week.) - hazelwood and bingham gardens' ecs worked together for the holiday bazaar, hot chocolate bar/doughnuts for staff and fundraised for the ronald mcdonald house. Both campuses made holiday treats in their adaptive cooking classes and sold them for $1 to donate. Hazelwood clients prepared peanut brittle and no-baked cookies. Bingham clients prepared cupcakes.- volunteering numbers increased for the quarter to 23 for hazelwood with the ronald mcdonald house.- board of health score of 100% for kitchen inspection on 12/11/24. - january 2025 - june 2025:- hazelwood got a new milnor washing machine in the laundry. - community homes residents participated in easter parade.- community homes hosted a music group with our new friends from down syndrome of louisville.- resident's artwork is on display at wilder caf in crestwood.- resident's artwork is on display at mt. Washington florist & coffee shop.- benefit concert for crusade for children at ba colonial with falls city freeloaders performing. - first spirituality group was hosted by the community homes at hazelwood.- the residents are making new items during sewing group at windsong. Currently they are making clothing protectors, small purses, and tote bags.- a resident conducted a workshop with cedar lake to teach them how to make keychains.- new enrichment center instructor hired at windsong.- it replaced 3 end-of-life network switches with new enterprise grade network switches. - it replaced 37 windows 10 computers with new windows 11 computers. - the transitions department successfully transitioned 1 individual during this timeframe, therefore meeting the contract goal. - respiratory replaced the tops to their five medication carts. - 245 appointments completed in the outpatient specialty clinic.- 6 new patients established in the clinic.- psychiatry scheduling for both outpatient specialty clinic and hazelwood/community homes improved.- noninvasive portable bladder scanner obtained to help decrease the risk of utis and hospitalizations due to neurosepsis.- transitions presented at bingham gardens resident council meeting on 7/18/25 to discuss the standard support needed, types of homes with pictures, and types of neighborhood settings with pictures. - a windsong community icf resident set up a table at the gaslight festival on september 13th and sold $280 in merchandise to benefit the crusade for children.- karaoke group in august at the ba colonial this is an activity that the residents enjoy, and we hope to continue to have the activity on a regular basis.- the therapy team attended the national q conference on august 6th and presented their relationships reimagined model. The response was positive, and they had lots of questions from participants afterwards. They also presented at hazelwood on august 26th.- survey completed at meadows and windsong with zero deficiencies. Del maria survey completed with one deficiency for expired food and one life safety deficiency for an open smoke barrier door in the attic. - windsong resident entered a quilt that she made in the state fair textiles exhibit and won a blue ribbon. Emily mcwilliams pta assisted her with the project.- meadows hosted 4th of july party on july 3rd for all community homes. - windsong had new flooring installed in one of the bedrooms. - september 24, 2025, the kitchen at hazelwood was inspected by the board of health and received a score of 98%! Minor deficiencies were immediately corrected. - hazelwood-the enrichment center wage hour division certificate has been renewed until 4/30/2027. The wioa (career counseling video, career counseling course quiz and survey questions) process was completed. Ky.gov sent the clients personal certificates but their names were misspelled. Now we are waiting for the corrected clients' certificates to arrive to have them placed in meditech.- hw-ec attendance progressed- june 76%, july 88%, and aug 100%- bg-ec attendance progressed-june 88%, july 83%, and aug 100%- hw and ch ec's entered art/craft items into the kentucky state fair and received participation ribbons.- hw ec conducted, formatted, and hosted a campus talent show.- hw ec clubs are experiencing/learning new things, taking community outings, and participating in activities affiliated with the clubs. - the ec dept. Received tickets to louisville slugger river bats game (sept 16th) from jennifer chappelle, metro council district 15 office, muhmmad ali center (oct 4th), and the louisville ballet co. Brown and forman nutcracker (dec 14th). All tickets will permit free entry to clients and their attending staff.- working with our collegiate partners, we had 3 pt students (1 from uk, 1 from saint louis university, 1 from bellarmine) working with us.- physical therapy developed a process to begin to use the home of the innocents' pool in august 2025 due to work being conducted on hazelwood's pool. This supports continued therapy for the residents who need it.- supported individual for spasticity management with outside physician.- trauma informed care surveys had positive growth noted.

Management of bingham gardens facility - major accomplishments include:-october 2024 - december 2024:- volunteering numbers increased for the quarter to 18 for bingham gardens with the ronald mcdonald house.- december 2024, a behavior analyst position was filled which marked the completion of filling the last vacancy in the clinical services department.- partnered with lyndhurst united methodist church, free of charge, to utilize space to conduct enrichment center (ec) activities until renovations are completed at lee specialty clinic's ec space. - january 2025 - june 2025:- january 2025 nutritional services purchased 6 medical grade weight scales for nursing.- january 2025 nutritional services purchased new dishwashers per maintenance.- the renovation of all tubs and showers was completed in all homes. (february 2025)- weekly rounding through the homes with the director and sodexo managers has resulted in improved cleanliness in the homes. - in the behavior department, the overhaul of challenging replacement skills training includes increased observations as well as increased modeling and staff training of implementation across all environments. This has resulted in increased mastery of replacement skills and reduction in challenging behavior for many of our individuals.- clients fundraised for the ronald mcdonald house by making holiday treats in their adaptive cooking classes and selling them at the holiday bazaar hot chocolate & doughnut bar. - ec department attended autism training from the university of louisville, kentucky autism training center. - client attendance to the enrichment center increased. - bingham gardens retained the qidps, social support coordinators, home managers, rncs and community home director with 0 turnovers. - zero turnover in the nursing department from october 24-december 24. - hired adon (june 2025). - new blockhouse furniture was purchased for the homes and the enrichment center. - the enrichment center and residential hosted a halloween party and chili cook-off. - the enrichment center received donated kentucky kingdom tickets for clients to attend christmas at the kingdom. - dietary facilitated several outings and activities: - outings to the pool - hosted several holiday parties for clients to enjoy (birthdays, easter, juneteenth, memorial day, super bowl, etc.) - threw an allergy friendly cookout for one of our clients' birthdays, who has 6 allergies to make him feel included. - we have created two completely revamped menus accommodating allergies on campus. - several clients have had a desired weight loss over the past 6 months. Completed diet education with several clients, especially the new one on campus. - conducted exercise learning objectives on a few clients with some success. - dietary moved their office to bingham gardens' campus to provide more support to staff and be readily available on campus more. - dietary advocated waffle makers or other items for cooking to help dsps and clients prepare food, especially for the clients with several allergies. They were purchased. - two clients participated in special olympics.
