Idd services:new vista of the bluegrass, inc. Serves as the primary assessment entity for all individuals seeking the michelle p. Waiver, a medicaid funded program, within our region. This system includes all initial assessments, as well as annual re-certifications for continued services. New vista of the bluegrass, inc. Continues to provide intellectual and developmental disabilities medicaid waiver case management services. Financial management is also provided to individuals who receive participant directed services. The medicaid waivers include supports for community living, michelle p, acquired brain injury and acquired brain injury - long term care. New vista of the bluegrass, inc. Idd services continue to focus on the needs of individuals with co-occurring behavioral health needs. New vista of the bluegrass, inc. Has a 24-hour, 7 day a week crisis response system that assists individuals with receiving support in the least restrictive environment and preventing in-patient hospitalization. New vista of the bluegrass, inc. Idd services also provides evaluators to respond to all 202b petitions for involuntary admission to an intermediate care facility that are presented in the new vista of the bluegrass, inc. Region. New vista of the bluegrass, inc. Also continues to provide initial evaluation and primary service coordination under the point of entry for the keis (kentucky early intervention system) program. This program serves children birth to 3 years of age, who are at risk of disability, or have a disability.new vista of the bluegrass, inc. Provides an array of services to individuals with idd through the community mental health center contract. These services include: access and referral, case management, subcontracted services to individuals who are on wait lists for medicaid waivers, specialized services coordination in nursing facilities through pasrr; and financial and technical support to affiliate providers of idd services within the region. The support provided to affiliates assists with increasing the availability of services throughout the region. Mental health (children):new vista of the bluegrass, inc. Is a certified community behavioral health clinic (ccbhc) providing a wide range of services to children, adolescents and families throughout the region in the form of care coordination, therapy, case management, psychiatric services, crisis services, supported employment, high fidelity wraparound, first episode psychosis (ihope) and targeted substance use treatment and psychoeducation for youth and families. New vista of the bluegrass, inc. Also has school-based services which include clinical supports, groups and psychoeducation to students as well as training opportunities for school staff.for children and adolescents in crisis, new vista of the bluegrass, inc. Provides a 24/7 response, meeting the individual in the community, school and via telehealth. Children and families utilize this service to prevent, divert or step down from hospitalization. Through a grant funded program in collaboration with the kentucky department of community based services (dcbs), new vista of the bluegrass, inc. Operates a sobriety treatment and recovery team (start) that provides service coordination to dcbs families affected by substance misuse to help determine service needs and link to resources. New vista of the bluegrass, inc. Supports families through high fidelity wraparound, which provides intensive supports and referrals to families with children with more intensive mental health needs. Youth peer support specialists are also available and work with children, adolescents, families and dcbs to engage as someone with lived experience. The early childhood mental health specialist and early childhood consultant provide training, consultation, and/or treatment to children birth to 5, families impacted by substance use and exposure as well as pregnant and parenting youth at risk for substance use disorder.new vista of the bluegrass, inc. Implements first episode psychosis (ihope) programming. Staff provide an array of wraparound services to individuals who are identified as at-risk for or currently experiencing psychosis, many of whom are in their teens or transition aged youth.new vista of the bluegrass, inc. Provides a wide array of evidence based practices. In addition to common modalities such as cognitive behavioral therapy (cbt), dialectical behavioral therapy (dbt), and motivational interviewing (mi), evidence based practices geared toward children and youth include, trauma-focused cognitive behavioral therapy (tf-cbt) parent-child interaction therapy (pcit), trust-based relational intervention (tbri), child-centered play, child-parent psychotherapy (cpp), multidimensional family therapy (mdft), cognitive behavioral interventions for trauma in schools (cbits), conscious discipline and family systems therapy.

Substance use disorder services:new vista of the bluegrass, inc. Is a certified community behavioral health clinic (ccbhc) providing a broad continuum of substance use disorder (sud) services for adolescents and adults including psychoeducation and prevention, case management, peer support, intensive outpatient programming (iop), outpatient therapy, medications for opioid use disorder (moud) and 28-day residential treatment for adults. New vista of the bluegrass, inc. Runs a licensed opioid treatment program (otp) and prescribes methadone, suboxone, buprenorphine and naltrexone mouds. New vista of the bluegrass, inc. Also operates quick response teams (qrts) that responds immediately following a non-fatal overdose.integrating treatment, new vista of the bluegrass, inc. Has primary care available for whole person care within the clinic setting. Care coordination works closely with primary care services to connect individuals with medical, psychiatric, clinical and social drivers of health resources. Individual placement and support (ips) supportive employment is also embedded in sud programming to provide more intensive support for those with a moderate or severe sud. Similarly, through utilization of the drop-in centers in fayette or madison county, individuals can access basic needs and housing services that can link to additional support, vouchers and/or affordable housing. Outpatient sud services serve clients of all ages with a primary diagnosis of sud. The bluegrass regional prevention center (rpc) continues to provide training and resources on underage drinking, neonatal abstinence syndrome and fetal alcohol spectrum disorders. New vista of the bluegrass, inc. Continues to work closely with the kentucky department of corrections as well as kentucky administrative office of the courts and the u.s. Probation office to provide treatment services for adults with substance use disorders throughout our region. Evidence-based practices utilized by new vista of the bluegrass, inc. Include dialectical behavior therapy, cognitive behavioral therapy, multidimensional family therapy, assessing and managing suicide risk, integrated dual disorder treatment, family psychoeducation, medication management, nurturing parenting programs, motivational interviewing, permanent supportive housing, individualized placement and support employment, person centered planning, seeking safety, matrix model, contingency management as well as partners for change outcomes management system (pcoms). Comprehensive training has been developed and implemented to support each of these initiatives. The orientation training program includes a comprehensive package of training designed to enhance staff competencies in all aspects of their job including delivery of evidence-based practices.

Primary care: new vista of the bluegrass, inc. Is a certified community behavioral health clinic (ccbhc) providing primary care to serve the region, with physical locations in all four of new vista's anchor sites (fayette, madison, scott and boyle counties) and one primary care provider collocated with the new vista of the bluegrass. Inc. Opioid treatment program. Telehealth is also utilized to increase the reach of primary care services. Recognizing the complex behavioral health and physical health needs of individuals served, new vista of the bluegrass, inc. Provides integrated care to target those most in need of care. Primary care services are offered to anyone without a current primary care physician. For clients who engage in primary care, new vista of the bluegrass, inc. Offers biometric screening, diagnostic assessments, treatments, and preventative care. New vista of the bluegrass, inc. Primary care providers also provide medication for the treatment of substance use disorders including moud (medication for opioid use disorder). Primary care providers are implementing health screening and monitoring for all clients in alignment with the whole person health philosophy of ccbhc. Care coordination works closely with primary care services to connect individuals with medical, psychiatric, clinical and social determinant of health resources.
