Programs

Professional Services helps provide comprehensive case management for guests needing health insurance including interpretive services, scanning, copying, and faxing documents, providing phone numbers to retrieve needed documents, and making calls on behalf of guests to get needed information and documentation. They also provide culturally responsive education and information about coverage options and how to best access care, including making referrals as needed. They also will assist in translating, reading, and explaining mail or other information received by the guest. NorthPoint assisted over 26,660 individuals through the use of the program.

Pregnancy and Family Health provides guests with both clinical and human services care concurrently, which facilitates consistent whole-person integrated care that meet the needs of the entire family. We also conduct routine outreach to increase attendance/enrollment, host information & enrollment sessions, host community-based events and activities focused on families and celebrating joy through the birthing journey, and work on other professional development needs. Key metrics include referrals, community engagement, financial support provided, and total number of guests served, which for the current year was 461 guests.

Youth Service Program is dedicated to a holistic approach to youth development. The Youth Program provides support and resources to youth with a focus on four pillars: school success, life skills development, physical health, and purpose development. During the year, the Youth Program helped 85 individuals. The Employment and Training Program at NorthPoint addresses the underrepresentation of BIPOC individuals in basic education, training and healthcare, training programs by offering support services, and connections to resources towards successful completion of training. Key components of the program include comprehensive case management, partnership development, participant recruitment and assessment, wrap around support services, career assessments, job training opportunities and career development focused internships for youth ages 16-24. NorthPoint assisted 308 individuals during the year. NorthPoint's Adult Program provides integrated, culturally responsive services including case management, care coordination, referrals, crisis intervention, advocacy, and recovery support to African American Men and Single Adults. Three key objectives include: improve employment and housing access, enhance social and cultural conditions, and reduce health disparities and promote well-being. The Adult Program helped serve 288 individuals during the year.
